How the Plans Work
You enroll for each of these benefits separately.
You purchase this coverage with after-tax payroll deductions, making benefits paid tax-free.
You can continue this coverage if you stop working at Conduent. You will need to pay premiums directly to Aetna.
These plans do not replace medical coverage and do not count as minimum essential coverage under the Affordable Care Act.
Critical Illness Insurance
Critical illness insurance pays a cash benefit directly to you if you are diagnosed with a covered critical illness, such as a heart attack or stroke. You can use your cash benefit to help pay bills related to treatment or to help with everyday living expenses. Benefits are paid regardless of any other medical insurance. You can purchase Basic or Enhanced coverage for yourself and your spouse. Dependent children are automatically covered.
Maximum Benefit Amount
Basic Plan: $15,000 for associate or spouse; $7,500 for children
Enhanced Plan: $30,000 for associate or spouse; $15,000 for children
Covered Illnesses
Examples of covered illnesses include:
Cancer
Heart attack
Major organ failure
End-stage renal (kidney) failure
Coronary artery bypass graft surgery
Stroke
Your initial benefit provides a lump-sum payment upon the first diagnosis of a covered condition. Your plan pays a recurrence benefit at 100% of the initial benefit for the following covered conditions: heart attack, stroke, major organ transplant, and end-stage renal failure.
Health Screening Benefit
This plan also offers a health screening benefit. You’ll be reimbursed up to $50 per year for each covered member if you get certain covered screening tests as outlined in the policy.
Accident Insurance
Accident insurance pays cash benefits to help with unexpected costs due to a covered accident. Aetna pays benefits directly to you to help pay bills or everyday living expenses. Benefits are paid regardless of any other medical coverage. You can elect the Basic Plan or the Enhanced Plan.
Covered Injuries
Examples of covered injuries include:
Broken bones
Burns
Torn ligaments
Cuts repaired by stitches
Eye injuries
Ruptured discs
Concussion
Ambulance
Family lodging
Emergency room treatment
Transportation
Hospital admission and additional benefits for confinement
Diagnostics (X-rays, CT, MRI)
Health Screening Benefit
This plan also offers a health screening benefit. You’ll be reimbursed up to $50 per year for each covered member if you get certain covered screening tests as outlined in the policy.
Hospital Indemnity Insurance
Hospital indemnity insurance pays cash benefits directly to you during a covered hospitalization, so you can avoid dipping into savings to cover out-of-pocket expenses. These expenses could include things like transportation and meals for family members, help with child care, or time away from work. You can elect the Basic Plan or the Enhanced Plan.
Benefits are paid regardless of any other medical coverage. Keep in mind, this benefit only pays when you are admitted as an inpatient and charged for room, board, and general nursing services.
Benefit Amounts
Basic Plan: $500 admission benefit¹; $100 daily hospital benefit (up to 30 days per calendar year)²
Enhanced Plan: $1,000 admission benefit¹; $150 daily hospital benefit (up to 30 days per calendar year)²
¹Lump sum benefit per hospital admission. Unlimited hospital admissions per calendar year are separated by 30 consecutive days.
²Daily benefit amount while inpatient in a hospital.
Example
For example, let’s say your child has a high fever and goes to the emergency room. The doctor admits your child to the hospital for two days. The Basic Plan would pay you $700 (hospital admission benefit of $500 and hospital confinement benefit of $100 per day).
